Hiring of Services of Chartered Accountant Firm / Cost Accountant Firm For Concurrent Audit of Districts Health Society (DHS) for the F.Y. 2016-17

Mission Director National Health Mission, Uttar Pradesh invites Expression of Interest (E0I) from eligible Chartered Accountant firms/Cost Accountant Firms for conducting monthly concurrent audit of the District Health Societies located at (Districts Azamgarh, Barabanki, Basti, Ballia, Bijnor, Ghazipur, Gorakhpur, Hardoi, Jaunpur, Kaushambi, Lakhimpur Kheri, Lalitpur, Maharajganj, Sitapur, Shahjahanpur, Sonbhadra) of Uttar Pradesh for the F.Y. 2016-17.

Eligibility Criteria The firm most be three year old. Applying audit firm should submit documentry proof for the above criteria. Technical and financial bid should be submitted in separate envelope. Both envelop will be kept in main envelop which will be supperscribed on the top of envelop “Proposal for concurrent auditor for Distt Health Society………………………….(Distt Name) UP.

The proposal documents containing detailed Terms of Reference (TOR) and application form can be downloaded from our website www.upnrhm.gov.in. Tender fee of Rs 500/- (Rupees Five Hundered) must be attached in the form of Crossed  Bank draft favouring “Districts Health Society, respective district,” payable at respective district headquarter along with the technical bids. Proposal documents without tender fee will be rejected. All the proposals should be dropped in tender box kept at CMO Office of the respective districts.

  1. Issue of proposal documents : 20.06.2016
  2. Last date for submission of Proposal to DHS : 04.07.2016 (Till 1.00 PM)
  3. Date of opening of technical bid : 04.07.2016 (At 4.00 PM)

Venue for opening of technical bids : Chief Medical Officer’s Office, “Respective District” UP.
